DHI Hair Transplant London: Cost and Who It Suits

Updated · By Hair Transplants London · 6 min read

Short answer

How much does a DHI hair transplant cost in London?

DHI usually costs about £900 to £1,200 more than FUE for the same number of grafts. In London that typically means around £5,800 to £6,100 for 1,200 grafts and £6,800 to £8,100 for 1,800 to 2,500 grafts, depending on the clinic and your plan.

Direct Hair Implantation, or DHI, is a variation of follicular unit extraction. The grafts are taken from the donor area in the same way as FUE, one by one. The difference is in how they are placed: instead of the surgeon making all the recipient incisions first and then inserting grafts, each graft is loaded into a fine hollow needle, often called a Choi implanter pen, and implanted directly in one movement. We offer DHI where it gives a genuine advantage for your hair and goals, and explain clearly when FUE would do the same job for less.

How does DHI work?

  1. Design: the hairline or treatment area is agreed and marked.
  2. Anaesthetic: local anaesthetic numbs the donor and recipient areas.
  3. Extraction: grafts are removed one at a time with a fine punch.
  4. Loading: each graft is loaded into an implanter pen.
  5. Implantation: the pen is inserted at the planned angle and depth, and the graft is released as it is withdrawn.
  6. Aftercare: dressing, medication and instructions.

Several pens are used in rotation so one can be loaded while another is used, which keeps the time grafts spend outside the body short.

Who is DHI best for?

DHI tends to suit:

  • Hairline work where precise angles and direction matter. See hairline transplants in London.
  • Adding density between existing hairs, because implanting without pre-made incisions can reduce disturbance to native hair.
  • People who want to avoid shaving the top, where an unshaven approach is suitable.
  • Small to moderate sessions, for example up to around 2,500 grafts.

FUE may be the better choice when:

  • A very large number of grafts is needed in one session
  • Cost is a priority and the result would be similar
  • The area is completely bald, where pre-made incisions are efficient

How much does DHI cost in London?

DHI is usually priced at a premium over FUE because implantation takes longer and needs more trained staff. Typical London prices:

Is DHI worth the extra cost?

It depends on what you need. For a dense, natural hairline or adding grafts between existing hair without shaving, many patients feel the premium is justified. For large bald areas where FUE can place more grafts efficiently, the extra cost may not add much. We will tell you at your assessment which technique we recommend for your case and why.

What is recovery like after DHI?

Recovery is very similar to FUE:

  • Days 1–3: redness and some swelling, possibly on the forehead.
  • Days 7–10: small scabs fall away with gentle washing.
  • Weeks 2–6: transplanted hairs shed; this is expected.
  • Months 3–4: new growth begins.
  • Months 12–18: final result.

Most people return to desk work after about a week. Our hair transplant recovery timeline covers each stage, and hair transplant aftercare explains washing, sleeping and activity.

Does DHI leave scars?

Extraction is the same as FUE, so DHI leaves tiny round marks in the donor area that are usually hidden by hair once it grows. There is no linear strip scar. See hair transplant scars for how FUE, DHI and FUT scarring compares.

How many grafts can DHI do in a day?

Many sessions place 1,500 to 3,000 grafts in a day, depending on the team and your hair. Larger cases may be split over two days or two sessions. The graft count needed depends on your stage of loss; see the Norwood scale and our hair graft calculator.

Unshaven DHI: what to know

An unshaven or partially shaven approach can make a transplant less noticeable in the first weeks, which appeals to people who cannot take much time away from work or social life. It usually suits smaller sessions and density work rather than large bald areas, and the donor area normally still needs trimming in a strip that can be covered by longer hair above it. Unshaven sessions can take longer, which is reflected in the price.

DHI for women

Women with thinning at the parting or frontal area often want to keep their hair long. DHI between existing hairs can suit this, sometimes without shaving the recipient area. See female hair transplants in London.

DHI in London vs abroad

DHI is widely marketed in Turkey and elsewhere at lower prices. When comparing, look at who performs each step, how many grafts are realistic, aftercare and what happens if you need follow-up. See hair transplants in the UK vs Turkey.

Questions to ask about DHI

  • Why do you recommend DHI rather than FUE for me?
  • Who extracts and who implants the grafts?
  • How many grafts can be implanted in one session for my case?
  • Does the recipient area need shaving?
  • What is included in the price?

DHI and graft survival

The main factors in graft survival are the same for every technique: how quickly grafts are implanted after extraction, how they are stored and handled, how gently they are placed and how well the scalp is looked after in the first two weeks. DHI keeps out-of-body time short because grafts are implanted as they are loaded, but careful FUE teams achieve the same with good workflow. What matters most is the skill and organisation of the team.

Is DHI more painful than FUE?

The experience is similar. Both use local anaesthetic, and most patients feel pressure rather than pain during implantation. Afterwards, soreness in the donor area is the same because extraction is identical. Some patients report slightly less swelling with DHI, but this varies between individuals.

What a DHI quote should include

  • Consultation, hairline design and graft estimate
  • Local anaesthetic, extraction and implantation
  • The implanter pens and consumables used on the day
  • Medication and aftercare products
  • Post-operative wash and follow-up reviews at key growth stages

Ask whether the price is fixed for a stated number of grafts or changes with the number placed on the day.

Is DHI a good choice for a first transplant?

For many first-time patients with early hairline or temple recession, DHI suits well, because it lets the surgeon place grafts closely in a small area without shaving the whole head. It is less often the first pick for very large sessions or advanced crown loss, where FUE with pre-made sites can be quicker and more economical. The right answer depends on your donor density, the area to cover and how much shaving you can live with. At an assessment the surgeon compares the techniques for your scalp and explains which one gives the best result for your budget.

Frequently asked questions

What does DHI stand for?
Direct Hair Implantation. Grafts are extracted individually, as in FUE, then implanted with a pen-like tool that creates the opening and places the graft in one step.
Is DHI better than FUE?
Not universally. DHI offers fine control of angle and direction and can be useful in dense or unshaven areas; FUE can implant more grafts per session and usually costs less.
Do you need to shave for DHI?
The donor area usually needs trimming. The recipient area can sometimes be left unshaven when adding density to existing hair.
How long does DHI take?
Usually one long day, often six to nine hours depending on the number of grafts.
When will I see DHI results?
The same timeline as FUE, with shedding in weeks 2 to 6, new growth from months 3 to 4, and final results at 12 to 18 months.
