# Scalp Micropigmentation London: Cost and Results

Updated 21 October 2026. By Hair Transplants London.

Short answer

## What is scalp micropigmentation?

Scalp micropigmentation (SMP) is a cosmetic tattooing technique that places thousands of tiny pigment dots in the scalp to look like short hair follicles. It creates the look of a close-shaved head, adds the illusion of density to thinning hair, or hides scars. It needs two to four sessions and periodic top-ups.

Not everyone with hair loss wants surgery, and not everyone is a suitable transplant candidate. Scalp micropigmentation, usually shortened to SMP, offers a different route. Using a fine needle, a practitioner deposits pigment into the upper layer of the scalp in dots that mimic the appearance of hair follicles. Done well, it gives the look of a full, closely shaved head; on longer hair, it reduces the contrast between hair and scalp so thinning looks less obvious.

| Use of SMP | What it does | Typical sessions | Who it suits |
| --- | --- | --- | --- |
| Full shaved look | Recreates a buzz-cut hairline and coverage | 3–4 | Advanced loss, Norwood 5–7 |
| Density illusion | Reduces scalp show-through under hair | 2–3 | Diffuse thinning, men and women |
| Scar camouflage | Blends FUT lines or FUE dots | 2–3 | Previous transplant patients |
| Post-transplant boost | Adds visual density alongside grafts | 2–3 | Transplant patients wanting more coverage |
| Alopecia areata | Disguises patches | Varies | Stable patches |

## How does scalp micropigmentation work?

1. **Consultation**: hairline design, pigment shade and expectations are agreed.

2. **First session**: a base layer of dots is applied, usually lighter and more spaced.

3. **Second session**: density is built up and the hairline refined.

4. **Further sessions**: shade and density adjusted to match your hair and skin.

5. **Top-ups**: every few years as pigment softens.

Each session typically takes two to five hours depending on the area. Unlike a standard tattoo, SMP uses specialised pigment and needles placed at a shallower depth to create crisp, follicle-sized dots.

## Who is SMP suitable for?

SMP tends to suit:

- Men with advanced hair loss who are happy with a shaved style

- People without enough donor hair for a transplant

- People who do not want surgery

- Men and women with diffuse thinning who want to reduce scalp show-through

- Patients with transplant scars they want to camouflage

It is less suitable if you want long hair in areas that are completely bald, if you have active scalp conditions, or if you are not comfortable with the commitment to top-ups.

## How much does scalp micropigmentation cost in London?

SMP is usually priced by treatment area as a package covering all sessions. Smaller areas such as scar camouflage cost less than a full scalp. When comparing quotes, check:

- How many sessions are included

- Whether the first top-up is included

- The practitioner's training and portfolio

- The pigment used and how it is expected to fade

For comparison, a hair transplant ranges from about £3,500 for 500 grafts to £8,900 to £11,900 for large sessions in London. See [hair transplant costs in the UK](https://hairtransplantslondon.com/guides/hair-transplant-cost-uk).

## SMP vs hair transplant

| | Scalp micropigmentation | Hair transplant |
| --- | --- | --- |
| Result | Appearance of follicles | Real, growing hair |
| Hairstyles | Shaved look; or density under existing hair | Most styles once grown |
| Donor hair needed | No | Yes |
| Downtime | Redness for a day or two | About a week for desk work |
| Time to result | Immediate after sessions | 12–18 months |
| Maintenance | Top-ups every few years | Medication may be advised for native hair |

Some people combine the two: a transplant for the hairline and front, and SMP to add density across a large area or blend a scar. Read [does a hair transplant work?](https://hairtransplantslondon.com/guides/does-a-hair-transplant-work) to understand what surgery can achieve.

## SMP for hair transplant scars

SMP is one of the most effective ways to disguise donor scars. For a FUT line, pigment dots are placed in and around the scar to match surrounding follicles. For over-harvested FUE donor areas, SMP restores the appearance of density between dots. See [hair transplant scars](https://hairtransplantslondon.com/guides/hair-transplant-scars) and [FUT hair transplants](https://hairtransplantslondon.com/guides/fut-hair-transplant-london).

## SMP for women

Women with diffuse thinning, a widening parting or traction alopecia can use SMP to reduce scalp show-through under longer hair. The technique uses softer, more diffuse placement than a shaved-look design. See [female hair transplants in London](https://hairtransplantslondon.com/guides/female-hair-transplant-london) for surgical options.

## What is recovery like?

- Redness for one to three days

- Avoid washing the scalp for a few days after each session

- No swimming, sauna, heavy sweating or sun on the scalp for about a week

- Keep the scalp moisturised once healed

- Use SPF on the scalp long term to slow fading

## How long does SMP last?

SMP is semi-permanent. Pigment softens over time from sun exposure, skin renewal and products. Most people need a top-up every few years. Sun protection makes a significant difference to how long results last.

## Possible risks

Risks are usually low with a trained practitioner but include colour shift if poor-quality pigment is used, dots spreading or blurring if placed too deep, an unnatural hairline design and allergic reaction to pigment, which is rare. Choose a practitioner who can show healed results, not just fresh ones.

## SMP and the Norwood scale

SMP can be used at any stage, but it is most often chosen at advanced stages where donor supply would not cover the area. See [the Norwood scale](https://hairtransplantslondon.com/guides/norwood-scale).

## Designing a natural SMP hairline

The hairline is what makes or breaks an SMP result. A natural design:

- Sits at a height that suits your age rather than a teenage hairline

- Has a soft, slightly irregular front edge instead of a sharp line

- Respects the natural temple points

- Uses smaller, lighter dots at the front edge and denser dots behind

- Matches the shade of your natural stubble, not a darker tone

Ask to see your proposed hairline drawn on before the first session, and take time to look at it from different angles.

## Can SMP be removed?

SMP can be lightened or removed with laser treatment, but it takes several sessions and is not always complete. That is why careful design and a conservative first session matter.

## How to prepare for SMP

- Shave or trim the treatment area as advised before each session

- Avoid alcohol and caffeine on the day if you are sensitive to discomfort

- Moisturise the scalp in the days before, but do not apply products on the day

- Avoid sunburn on the scalp in the weeks before treatment

- Bring a hat for the journey home

## Living with SMP

Day to day, SMP needs very little maintenance beyond what you already do. If you keep a shaved style, you will need to shave regularly so your stubble matches the pigment. Moisturising helps the scalp look healthy, and a high-factor sun cream or hat protects the pigment outdoors. Exfoliating scrubs and harsh peels should be avoided because they speed up fading.

## SMP questions to ask a practitioner

- Can I see healed results, ideally a year or more old?

- How do you choose the pigment shade for my skin tone?

- How many sessions are included, and is the first top-up included?

- How will the hairline be designed for my age and face?

- What happens if I want to change the design later?

## Combining SMP with a hair transplant

Some patients use scalp micropigmentation alongside a transplant rather than instead of one. SMP can add density behind transplanted hair, soften the look of a strip scar from FUT, or make thinning areas less noticeable while grafts grow. If you plan both, the usual approach is to have the transplant first and let it settle before SMP, so pigment is not placed where grafts will later grow. Tell both practitioners about your plans, because the order and timing affect the final result.

## Is SMP right for you?

At an assessment we look at your hair loss, donor supply and goals, and explain whether a transplant, SMP, non-surgical treatment such as [PRP](https://hairtransplantslondon.com/guides/prp-hair-treatment-london), or a combination is most likely to give you the result you want. ## Frequently asked questions

### How long does scalp micropigmentation last?

Results typically last several years. Pigment gradually fades, and most people have a top-up every few years to keep the colour and density.

### How many SMP sessions do I need?

Usually two to four sessions, about one to two weeks apart, to build up density and match the hairline.

### Does SMP hurt?

Most people describe it as uncomfortable rather than painful, similar to a light tattoo. Sensitivity varies across the scalp.

### Can SMP hide a hair transplant scar?

Yes. SMP is commonly used to camouflage FUT strip scars and FUE dots in the donor area.

### Is SMP better than a hair transplant?

They do different things. A transplant grows real hair; SMP creates the appearance of hair follicles. SMP suits people happy with a shaved look or who lack donor hair.
